A fourth-trimester support plan

A useful fourth-trimester plan names who owns everyday work, how the recovering parent can ask for help, how rest and appointments will be protected, and where to go for medical or emergency care. Fill it in together before birth, then revise it after you come home.

Build your support plan

Fill this in together. For each job, agree who will handle it, who can step in, and when it needs to happen. Come back to the plan as your needs change. Leave anything you would rather keep off paper blank.

What you write stays on this page. It is not saved or sent to analytics. Refreshing the page, leaving it, or changing languages may clear your entries. Print your plan or save it as a PDF before leaving, and keep your copy private.

What is the fourth trimester?

“Fourth trimester” is an informal name for the early period after birth, not a fixed medical stage or a deadline for recovery. WHO’s global postnatal-care guideline focuses on the first six weeks after birth, while ACOG’s U.S. guidance describes postpartum care as an ongoing process through a comprehensive visit no later than 12 weeks and continued care as needed. Your own follow-up should match the plan from qualified professionals where you live.

Medical and emergency boundary

This plan organizes support; it does not tell you whether a symptom is normal. We will review the CDC’s urgent maternal warning signs. If an urgent warning sign occurs, we will seek immediate medical care and state that the person is pregnant or was pregnant within the last year. For immediate danger, we will use local emergency services—not a message, app request, or email.

  • We reviewed our clinician’s discharge and follow-up instructions.
  • Both of us know where those instructions and contact details are kept.
